The Community Health Team (CHT) provides services statewide with a focus on Providence, Central Falls, Pawtucket, Cranston, Lincoln, East Providence, North Providence, Johnston, and Smithfield – most some of the hardest hit during the pandemic and often the most underserved. CHT works with babies, children, adolescents, adults, and families with high-level medical and behavioral health needs, providing critical and often-overlooked patient advocacy, care coordination, interpretation services, and basic needs assistance.
